Removed Pitcher returning to Pitch?
3-1-2 States that a pitcher may be removed as pitcher and returned as pitcher only once per inning, provided the return as pitcher does not violate either the pitching, substitution or charged conference rule.
What does the conference rule have to do with a pitcher returning to pitch?

Is this standard practice? I do not think I have a seen a coach re-insert a pitcher in a high school game? Is this just my inexperience?

This is not done very often. Usually this occurs when after the first inning or so the team has a large lead. They remove their Ace pitcher, keeping him in the game at another position. Then when the other pitcher is in trouble and they need the Ace to close the game out, they return him to the mound.
This is usually done to save the ace for when more important or cruical innings.
So to answer. Standard practice? no its not done very often. This is used
just in certain situations.

What does the conference rule have to do with a pitcher returning to pitch?
Say a team has already had three charged conferences. The coach comes out and swaps the left fielder and the pitcher. The player could not then return to the mound to pitch -- his removal as pitcher was required by the rules since the team had reached the allowed number of charged conferences.
I would think that the charged conference rule would apply in little league rules. Each manager is allowed 4 conferences per pitcher. The question would then be, if the starting pitcher was removed during an inning would that trip count as a conference?
